Membership is £50 per year (£25 if within the first 5 years of practice). Membership year starts 1 November annually.

Print out the Application Form  and Standing Order Instruction.

Send the Application form to the address shown on it (or email it to Peter Andrews). 

Fill in your details on the Standing Order Instruction and edit the start dates as appropriate for your date of application (put your name in the Payment Reference of Payee Details). Sign and date it and post it to Simon Reevell at 39 Park Square North, Leeds LS1 2NU (DX 26407 Leeds)

If you do not wish to set up a standing order, forward a cheque for £50 (or £25 if applicable) to Simon Reevell. Please write the names of the individual members covered on the rear of the cheque.
